Analisis Utilitas Biaya Annual Population-Based Screening Dibandingkan dengan Opportunistic Screening Diabetes Melitus di Indonesia Menggunakan Markov Model

Abstract


Prevelensi Diabetes Melitus (DM) di Indonesia semakin meningkat dari tahun ke tahun dan berdampak pada tingginya biaya perawatan. Salah satu upaya pencegahan DM yang dapat dilakukan adalah melalui deteksi dini atau skrining. Saat ini di Indonesia menerapakan oportunistic screening dan tidak menerapkan population-based screening. Penelitian ini bertujuan untuk melihat perbandingan efektivitas biaya oportunistic screening dan population-based screening DM di Indonesia serta menganalisis faktor yang paling berpengaruh terhadap nilai incremental cost-effectiveness ratio (ICER). Penelitian ini dilakukan di Universitas Padjadjaran pada november 2020 hingga maret 2021. Nilai efektivitas biaya dihitung berdasarkan Markov model dengan siklus 1 tahun dalam time horizon 19 tahun.  Data yang digunakan sebagai input parameter adalah data epidemiologi, biaya (payer perspective) dan utilitas (QALYs). Data yang diperoleh kemudian dianalisis dengan mempertimbangkan probabilitas transisi antar health states. Hasil ICER akan dibandingkan dengan Produk Domestik Bruto (PDB) perkapita sebagai cost-effectiveness threshold. Population-based screening memiliki estimasi total biaya Rp8.530.479 per 13,768 QALYs dan oportunistic screening memiliki estimasi total biaya Rp7.115.974 per 13,743 QALYs. Nilai ICER adalah Rp79.502.211 dan nilai PDB perkapita adalah Rp56.938.723. Dapat disimpulkan bahwa population-based screening DM di Indonesia masih cost-effective apabila menggunakan cost-effectiveness treshold 1-3 PDB perkapita. Analisis sensitivitas menunjukkan bahwa biaya skrining, kualitas hidup pasien DM komplikasi dengan early maupun late treatment merupakan faktor yang paling berpengaruh terhadap nilai ICER.
